FNP/PA Program:
Medical Entrance Requirements

For your health and safety and for the safety of your colleagues and patients, the following vaccinations and screening procedures are required upon admission to the UC Davis FNP/PA Program. You must meet the following health requirements, since you will be exposed to clinical situations during the very first week of classes. You will not be allowed to begin your classes and attend any community clinics until you are cleared.

Student Health and Counseling Services must receive all documentation by June 1, 2011. Those who fail to meet clearance prior to entry or upon arrival to campus are subject to holds on enrollment, classroom/clinical activity, and/or financial aid as determined by the UC Davis FNP/PA Program.

Mandatory Immunizations & Records (required)

To assist you with the collection of your immunizations and records, your personal physician/provider can fill-out and sign the Entrance Requirement Form (pdf) as proof of completion. The required information listed below can either be submitted on this form or attached as lab results and immunization records.

All mandatory items must be submitted to receive medical clearance:

Medical records documentation of a positive immune antibody TITER test for:

  • Measles (Rubeola)
  • Mumps
  • German Measles (Rubella)
  • Chickenpox (Varicella): by IgG immune status verification
  • Hepatitis B: by quantitative HBS Antibody immune status verification

There are different titers available for Chickenpox and Hepatits B. Make sure that you specify the required titer to your healthcare provider.

**Vaccination records do not substitute for titers.**
Students with negative or inadequate antibody titer levels for measles, mumps, rubella, and chickenpox are required to provide additional documentation of primary vaccination series as well as booster received after negative/inadequate titer date. Students with negative or inadequate antibody titer levels for hepatitis B should contact Ana Lopez at Student Health and Counseling Services using Health-e-Messaging.

Medical records documentation of tuberculosis screening by PPD testing:

Submit a copy of the test, with the date and location/provider stamped or signed by an authorized health care agent indicating:

  • a negative result performed after February 2011 OR
  • a positive result performed any time in the USA with a chest x-ray performed after July 2010 demonstrating absence of active disease

Persons who have completed INH prophylaxis are encouraged to provide documentation of completed treatment for their medical files.

Recommended Immunizations (optional)

SHCS strongly recommends that you consider submitting medical records documentation of vaccinations for the following: 

  • Tdap (tetanus, diphtheria and pertussis) - The California Department of Public Health recommends an adult pertussis booster vaccination. All health care personnel, particularly those who have contact with infants and pregnant women, should be immunized with Tdap to protect their patients and themselves.
  • Polio (IPV or OPV) vaccinations
  • Hepatitis A vaccinations
  • any documentation of immunizations related to travel prevention or occupational disease prevention

Some of these vaccinations may be required documentation in your future years of education at UC Davis.

If you have met the clearance requirements, we will contact you using the Health-e-Messaging system once we have reviewed your submission. You will also be notified through Health-e-Messaging if there are any problems with your submission. If your materials are submitted on time, you should expect to hear from us around the middle of June - please do not call. If you have any questions about the requirements, you can contact Ana Lopez at Student Health and Counseling Services using the "Messages" option in Health-e-Messaging. Create a "New Message" and select the option to "Ask an Administrative Question." This messaging system can also be used to ask a question if you are unsure about a requirement.

Persons whose unique medical history or religious practices prohibit immunization or persons with any questions or concerns regarding these requirements should contact Thomas Ferguson, M.D., Ph.D., Medical Director using Health-e-Messaging.
