Secure Messaging: Using Health-e-Messaging

As an FNP/PA student, your Medical History Form and Respirator Clearance Form need to be completed in our secure messaging system, also know as Health-e-Messaging. You can also use Health-e-Messaging to ask questions about the UC Davis medical entrance requirements and check the status of your clearance. This free service is a safe and confidential way to communicate about your non-urgent healthcare needs. Only you and SHCS staff will have access to your private healthcare information. In order to take advantage of this secure electronic messaging system, you will need to have set-up your UC Davis LoginID and kerberos password. You will also need your student identification number. If you are unsure of what these are or do not have one yet, please contact your school or department to obtain.

Medical History Form Instructions

Once you have logged into Health-e-Messaging, go to the "Forms" section by using the link in the left-hand navigation bar or center of the page. From the "Forms" page, select and complete the "Medical History Form." Please note that you must complete this form in one session. The form will not allow you to save partial entries and entries cannot be modified after submitted.

Respirator Clearance Form Instructions

Log into Health-e-Messaging and go to the "Messages" section by using the link in the left-hand navigation bar or center of the page. From the "Messages" section, create a "New Message" and select the option to "Request Respirator Clearance." Fill out the form and sumbit.

Checking the Status of your Clearance or Ask a Question

If you have met the clearance requirements, we will contact you using the Health-e-Messaging system once we have reviewed your submission. You will also be notified through Health-e-Messaging if there are any problems with your submission. If your materials are submitted on time by the June 1 deadline, you should expect to hear from us around the middle of June - please do not call. If you have any questions about the requirements, you can contact Ana Lopez at Student Health and Counseling Services using the "Messages" option in Health-e-Messaging. Create a "New Message" and select the option to "Ask an Administrative Question." This messaging system can also be used to ask a question if you are unsure about a requirement.

Additional Secure Messaging Features

Once the school year begins, you will also be able to use the Health-e-Messaging system to contact Student Health and Counseling Services regarding other non-urgent healthcare needs:

  • Follow up on your visit, by communicating with your provider
  • Request a prescription refill
  • Complete your online Medical History form
  • Fill out pre-appointment visit questionnaires
  • View or cancel your scheduled appointments
  • Ask about Medical Entrance Requirements
  • Ask a question about your insurance referral
  • Request a medical records release for an outside provider
  • View and print billing statements

You will be prompted to provide your UCD LoginID, kerberos password, and student ID.
