Where and When to Submit a Waiver Application
(Academic Year: Fall 2007 - Summer 2008)
How long is my waiver effective?
All approved waivers are effective beginning with the term for which you have applied and remain effective for the duration of the current academic year. If you want to waive participation in SHIP for the next academic year, you will have to file another waiver application.
When must I submit my waiver application?
Be sure to submit your waiver application or payment IN ADVANCE of the fee payment deadline to avoid a late fee. Links to the registrars' websites are available below for fee payment deadlines:

Waiver applications must be received by these dates in order to apply to the quarters indicated (NO EXCEPTIONS):

For some student groups, SHIP waiver application deadlines are different from those listed above, based on the dates that their academic programs begin. Examples of this are FNP/PA, LLM, MPVM, and 1st year Veterinary students. Check the information for your student group on the new students page if your academic program begins on a different date than the beginning dates of coverage listed above.
Where should I submit my waiver application?
You may apply online for a SHIP waiver beginning in May of each academic year. If you are unable to apply online, you may download a paper waiver application and submit it to Insurance Services at Student Health Services. The waiver application deadlines for the 2007-2008 academic year have passed.
