SHIP Enrollment Form
All UC Davis registered students are automatically enrolled in SHIP and are not required to submit an enrollment form. This enrollment form is intended primarily for students going on PELP or Filing Fee status or in the first term following graduation. Eligibility After A Registration Status Change
Whenever your registration status changes, your existing SHIP coverage and your eligibility for SHIP is impacted. Read this important information if you anticipate that your registration status will be changing, e.g., if you plan to graduate, go on PELP or Filing Fee, withdraw or cancel registration, or are dismissed:
